Players must pre-register for tryouts by filling in this form. No walk-ups will be allowed! $25 Tryout fee to be paid online before first tryout. Tryout fee covers all tryouts that the player attends. Tryout dates will be posted on our website once they are determined (early September). In the event of inclement weather please check the website for any changes in the schedule. Players must bring gloves, cleats, helmets, athletic cup and water as well as wear appropriate baseball clothing.

Tryout Information

Tryout fee must be paid online by Credit Card prior to the deadline.
  1. Release of Liability

    I, the parent/guardian of the named candidate(s) for a position on an Orangeville & Headwaters Minor Baseball (OHMBA) team, hereby give consent to participate in any and all OHMBA activities. I assume all risk and hazards incidental to such participation including transportation to and from the activities. I do hereby waive, release, absolve, indemnify and agree to hold harmless the totality of the OHMBA, Ontario Baseball Association, the organizers, sponsors, supervisors, participants and persons transporting my child(ren) to and from activities and any claim arising out of an injury to my child(ren) or by any acts or omissions of this/these child(ren) causing damage to any property or injury to any person, whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident and liability insurance.

    I also acknowledge that I understand that there will be no refunds issues for Tryout fees.

    I further agree to abide by the rules, regulations and bylaws of Orangeville & Headwaters Minor Baseball Association, and consent to the use of any and all private information herein provided in accordance with the Personal Information Protection and Electronics Document Act (PIPEDA) in this year or any successive years of participation.


  3. Photo Consent

    I hereby grant permission to Orangeville & Headwaters Minor Baseball Association (OHMBA) to use the images of my registered players below on its Web site and Social media platforms without further consideration. I understand that no names will be used on the web site unless specific permission, in written form, is given to an appropriate staff member. 

    I understand that once any image is posted to the OHMBA web site or social media platforms, the image can be downloaded by any computer user around the world. I hereby attest that I am the legal parent/guardian of the child(ren) listed below. This consent is effective until such time as I revoke it in writing and provide a copy of the revocation to the OHMBA. 

  4. Rowan's Law Acknowledgement:

    Rowan’s Law was named for Rowan Stringer, a high school rugby player from Ottawa, who died in the spring of 2013 from a condition known as second impact syndrome (swelling of the brain caused by a subsequent injury that occurred before a previous injury healed). Rowan is believed to have experienced three concussions over six days while playing rugby. She had a concussion but didn’t know her brain needed time to heal. Neither did her parents, teachers or coaches.

    Rowan’s Law and Rowan’s Law Day were established to honour her memory and bring awareness to concussions and concussion safety.

    I hereby confirm that: (1) the player being registered to participate in baseball activities with my local association and Baseball Ontario, and the parent or legal guardian of the player being registered if the player is under 18 years of age, have reviewed one of the concussion education resources provided by the Province of Ontario as referenced above and (2) agree to comply with the player code of conduct(s) as referenced above. I / we further acknowledge understanding the nature and risk of concussion and head injury to athletes, including the risks of continuing to play after a concussion or head injury is suspected. 


Human Validation
